Breast Intervention

  • Health Coverages and Medical Fees
  • Civil Servant Medical Benefits Scheme (CSMBS) by the Comptroller General’s Department
    1. Obtain a diagnostic request form from the doctor.
    2. On exam day, insert a national ID smart card into the verification machine at service points on 1st floor of the OPD Building or 1st floor of 100th Year Somdech Phra Srinagarindra Building, or near eleva-tors on 7th floor of 100th Year Somdech Phra Srinagarindra to verify eligibility.
  • Other government coverage
    1. Obtain a diagnostic request form from the doctor.
    2. On the day of the examination, please bring the diagnostic request form to Thanyarak Breast Center.
  • Social Security Scheme (Siriraj Hospital)
    1. Obtain a diagnostic request form from the doctor.
    2. On the day of the examination, proceed to Medical Records Unit on the 1st floor of the OPD building (Room 100) or Social Security Unit on the 3rd Floor of Navamindrapobitr 84th Anniversary Building with the diagnostic request form to verify the eligibility.
    The diagnosis request form without the “Special After Hour Clinic” stamp will not be eligible for any appointment at outside official working hours, in regards to the Social Security Scheme.
  • Social Security Scheme (Other hospitals)
    1. Obtain a diagnostic request form from the doctor.
    2. Contact a registered Social Security Hospital with the request form to obtain a referral letter.
    3. On the day of the exam, go to Medical Records Unit on the 1st Floor of the OPD building (Room 100) or Social Security Unit on the 3rd Floor of Navamindrapobitr 84th Anniversary Building with the diagnos-tic request form and referral letter to verify the eligibility.
    The diagnosis request form without the “Special After Hour Clinic” stamp will not be eligible for any appointment at outside official working hours, in regards to the Social Security Scheme.
  • Universal Coverage Scheme or Gold Card (Siriraj Hospital)
    1. Obtain a diagnostic request form from the doctor.
    2. Contact Siriraj Primary Care Unit, Faculty of Medicine Siriraj Hospital with the diagnostic request form to obtain a referral form.
    3. On exam day, bring the diagnostic request form and referral form to the registration unit, Health Ben-efit Division, Room 101, 1st floor of the OPD building to verify your eligibility. Then, insert your nation-al ID card into the verification machine located at service points on the 1st floor of the OPD Building, 1st floor of the 100th Year Somdech Phra Srinagarindra Building, or near elevators on the 7th floor of the 100th Year Somdech Phra Srinagarindra Building to receive a confirmed verification slip.
    Remark: The referral form must specify “breast disease” to obtain this coverage scheme. The diagnosis request form without the “Special After Hour Clinic” stamp will not be eligible for any appointment at outside official working hours, in regards to the Universal Coverage Scheme.
  • Universal Coverage Scheme or Gold Card (Other hospitals)
    1. Obtain a diagnostic request form from the doctor.
    2. Contact your registered Hospital with the diagnostic request form to obtain a referral letter.
    3. On the day of the exam, bring the diagnostic request form and referral form to the registration unit, Health Benefit Division on the 1st floor of the OPD building (Room 101) to verify your eligibility. Then, insert your Thai National ID card into the verification machine located at service points on the 1st floor of the OPD Building, the 1st floor of the 100th Year Somdech Phra Srinagarindra Building, or near elevators on the 7th floor of the 100th Year Somdech Phra Srinagarindra Building to receive a con-firmed verification slip.
    Remark: The referral form must specify “breast disease” to obtain this coverage scheme. The diagnosis request form without the “Special After Hour Clinic” stamp will not be eligible for any appointment at outside official working hours, in regards to the Universal Coverage Scheme.
  • Universal Coverage Scheme or Gold Card (Inactive Status)

  • If the National Health Security Office (NHSO) terminates a hospital's contract, making it inactive, the patient can still receive free care at the primary care unit of another hospital. This will continue until the National Health Security Office (NHSO) assigns a new contracted hospital to the system for patient registration.

    On the day of the exam, the patient needs to bring the diagnostic request form and referral form to the registration unit, Health Benefit Division on the 1st floor of the OPD building (Room 101) to verify your eligibility. Then, insert your Thai National ID card into the verification machine located at service points on the 1st floor of the OPD Building, the 1st floor of the 100th Year Somdech Phra Srinagarindra Building, or near elevators on the 7th floor of the 100th Year Somdech Phra Srinagarindra Building to receive a confirmed verification slip.
  • Non-reimbursable Payment
    1. Obtain a diagnostic request form from your doctor and schedule an appointment at the center.
    2. Come in person at your scheduled appointment date and time.
  • Examination Fee

  • The examination fee depends on the type of breast intervention procedure. The nursing staff will assess and inform each patient individually, as each individual’s examination procedure is different.

  • Remark

  • • A THB 100 charge for OPD service will apply for all coverage types.
    • For after-hours procedures, an additional THB 400 charge per visit at Thanyarak Breast Center will ap-ply, as determined by Siriraj Hospital.
